Four steps, always with the case manager at the helm: a home assessment, a care plan, team coordination and a report every week.
Assessment at home
The case manager comes to your home. The assessment is €45 and is credited against your first month if you go ahead. They assess the person and the environment using validated clinical indices, listen to the family, and return a report and a proposed plan.
Care plan
Together we define what is needed, how often and by whom: case manager visits, clinical procedures, therapies, care technician hours. Everything in writing, every price known upfront.
Coordination
The team comes into the home and the case manager coordinates: scheduling, liaison with the hospital, adjustments to the plan, response to complications. The family stops making phone calls.
Weekly report
Every Friday, by email: what happened, how things are evolving, and what comes next week. In clear language.

Booking the assessment
We arrange the case manager's visit to the home.
The plan
You receive the written plan, with the price known upfront.
Your family's place is reserved for launch
The team comes into the home when the family decides to go ahead.
The case manager has three packages: Essential €90 / month, Integral €160 / month and Premium €240 / month. Clinical acts and care-technician hours are set after the home assessment, with the price known upfront and a 10% discount on the à-la-carte table. Home assessment: €45. If you take a package, it is deducted from the first month.
