Responsibility
There is always a name, a face and a phone number responsible for your relative's case.
Rigour
We assess with the same clinical indices used in hospital and long-term care services, and we show the evolution, week by week, in language you can understand.
Transparency
You know what happened, who was in the house, what it cost and why. One report a week. One invoice a month.
The central figure of the model
One person answers for everything. And they have a name.
From day one, your family has a case manager, a nurse or doctor, who knows your relative by name, knows the medication, knows the context and knows the history. They build the care plan, carry out the scheduled visits, liaise with the attending doctor and the hospital, and coordinate the whole team that enters your home. And every week, they sign the report you receive.
It is the difference between having services at home and having coordinated care at home.
The monthly fee is the same whether the case manager is a nurse or a doctor. The choice depends on the clinical complexity of the case, never on price.
